Requesting Records/Results

If you would like to request a copy of your medical records, you must fill out a HIPAA form so our office can provide you with your records. This form must be filled out in its entirety and signed by the patient or legal guardian. Please keep in mind that our Privacy Officer must sign off and review this form before the records can be released. According to HIPAA guidelines, our office has at least 30 days to respond to your request, so you may not receive your copies immediately.

It is the policy of Neera Agarwal-Antal, M.D., Inc. to only provide biopsy and other test results to patients at the time of their follow-up appointment. No results are given over the phone. The purpose of this policy is to:

  • Ensure that our patients’ medical records are not released to any unauthorized individuals.
  • Ensure that enough time is allowed for the patient to ask questions, understand their results, and discuss the option for further treatment if necessary.
  • Ensure that the appropriate staff and accurate information are provided to the patient.
