Notifications and Test Results

We prioritize your convenience and privacy when sharing test results. Notifications will be sent via phone, mail, or SMS text message. Please note that standard messaging rates may apply.

For biopsy and test results, we share findings during follow-up appointments to ensure you have ample time to ask questions, understand the results, and discuss next steps with our team.

Update Your Patient Information

Stay up to date! Please complete the Patient Registration Form to update your details. Bring a signed copy to your next visit or fax it to our office.

Requesting Records and Results

If you would like to request a copy of your medical records, you must fill out a HIPAA form so our office can provide you with your records. This form must be filled out in its entirety and signed by the patient or legal guardian. Please keep in mind that our Privacy Officer must sign off and review this form before the records can be released. According to HIPAA guidelines, our office has at least 30 days to respond to your request, so you may not receive your copies immediately.

It is the policy of Neera Agarwal-Antal, M.D., Inc. to only provide biopsy and other test results to patients at the time of their follow-up appointment. No results are given over the phone.
